Case:
40 year old female with 15 year old subglandular (above the muscle) 295 cc saline implants. Baker grade 3 capsular contracture on the right with bottoming out or inferior implant displacement on both sides. The nipples have moved upwards and outwards becuasue of the implants moving downwards. She denied any pain or discomfort. She wanted to correct the deformity and exchange the implants to silicone. A pocket change was performed below the muscle, 305 cc smooth round silicone implants were placed subpectorally (below the muscle). The old capsules were removed (total capsulectomy) to treat the capsualr contracture and ADM (acellular dermal matrix) mesh reinformeent was performed at the lower poles to support the thin tissues and inframammary fold (IMF). This will help prevent bottoming out again and support the implants. A breast lift (mastopexy) was performed to remove the excess skin that was created by the original implants and deformity.
The patient is very happy with softer more natural symmetrical looking breasts. She is seen here with a very early result with steristrips on the incisions. The incisions will continue to heal over the next few months.
